Services Provided

Our support coordination services can help you get the most out of your NDIS funding package. Following your NDIS plan approval, our support coordinators will work with you to identify your personal support needs and ensure you are connected with the right service provider.

We will check in with you throughout the year to ensure your support providers meet expectations and if you are on track with your goals. But we are available at any time to step in and rectify issues you may have, including making service agreement changes or helping you find a replacement service provider.

We will connect you with the right provider and service for your circumstances - choosing Heart2Heart for support coordination does not mean you will or must receive support from our team. As ever, the choice and control rest in your hands.
